Febrile and Afebrile Seizures associated with Mild Gastroenteritis: A Clinical Comparison with Febrile Seizures

Abstract
Purpose: Febrile seizures and afebrile seizures associated with mild gastroenteritis are neurological disorders common among infants and young children. We evaluated the clinical characteristics of febrile and afebrile seizures associated with mild gastroenteritis with those of non-gastroenteritis-related febrile seizures.

Methods: We retrospectively reviewed the medical records of 678 children aged 3 months to 5 years, presenting with febrile seizures associated with acute gastroenteritis (Group A, n=97), afebrile seizures associated with mild gastroenteritis (Group B, n=30), and febrile seizures in the absence of gastroenteritis (Group C, n=551). We compared the mean age, sex, seizure semiology, frequency, clustering, duration, family and prior history of febrile seizures, stool rotavirus antigen, and electroencephalographic (EEG) findings among the three groups.

Results: The mean age, sex ratio, and seizure semiology were similar among the three groups. Generalized seizure was the most common type of seizure in all groups. More Group B than Groups A and C patients experienced ¡Ã2 seizures/day (P= 0.002). Group A and C experienced more prolonged seizures, lasting ¡Ã5 min, than did Group B (P=0.023). A family history of febrile seizures was more prevalent in groups A and C than in Group B (P=0.043).

Conclusion: Patients with febrile seizures (Groups A and C) had several features distinct from those with afebrile seizures associated with mild gastroenteritis (Group B). However, clinical manifestations did not differ according to the presence of absence of gastroenteritis symptoms in patients who had febrile seizures.
